Improving Access To and Quality of Sexual and Reproductive Health Services in
Kampong Speu Province, Cambodia

1. Introduction
Since March 2011, E&D has been implementing a Sexual and Reproductive Health (SRH) Project in target areas of Kampong Speu province. The overall scope of Implementation includes the Provincial Health Department (Mother and Child Health Care); Provincial Referral Hospital-Kompong Speu, Referral Hospital Kong Pissei and 35 Health Centers.This project has two main components: prevention and medical components.For the prevention component, “Intensive Community support areas” includes Health Operational Districts (OD) of Oudong (1 commune) and Kampong Speu OD (8 communes of Phnom Srouchh and Oral administrative districts). The Project “Improving Access to and Quality of Sexual and Reproductive Health Services in Kampong Speu Province, Cambodia” seeks to increase the quality of care at Referral Hospitals and 35 Health Centers of Kampong Speu province. The Project aims to increase the capacity of public health workers and to identify and empower community volunteers to work with women of child bearing age to address key issues related to gender based violence and sexual and reproductive health.

To ensure sustainability of project community level initiatives, it is proposed that a consultant be hired to assist the identification of existing community interest groups at village and commune levels, structure them into community based organizations, and empower them with basic skills to function independently of NGOs. It is envisioned that by the end of the consultancy, the community groups would have basic skills to do needs assessments, plan, network, and fundraise for community initiatives, implement activities, and monitor/evaluate the impact of the interventions. Existing village based groups in the current project sites include doulas  and social workers who are already trained and functioning to address key issues related to gender and sexual/ reproductive health.

2. Background
Kampong Speu province is among the poorest provinces of the country and faces problem of availability and accessibility of medical care in general and of access to Basic and Comprehensive Emergency Obstetric and Neonatal Care (CEmONC) in particularly. Maternal mortality rate is very high in rural areas with a significant gap with the urban. There is a lack of in-service trainings, follow-ups after trainings, supportive supervisions and coaching. Health Centre midwives lack sufficient skills. There are problems of infrastructure and equipment/drugs and supplies and transportation systems. These barriers are caused by the far distance from the home to the health services, the availability and cost of the transport, wrong and sometimes harmful traditional beliefs and practices around maternity etc. Births attended in public health facilities remains around 44% (2009). The use of modern contraceptives is approximately 26%, and 8% of women age 15 to 49 reported having had one or more abortions in their lifetime, mostly at home with the consequent medical risks.

Public health services in remote locations of Kampong Speu province are very rudimentary and the major obstacle to finding professional help is linked to the accessibility of quality obstetrical health services. The nearest obstetrical services for pregnant women with complications are at the Provincial Referral Hospital, where special interventions are available.
Among other achievements of the current project, until now, 73 village based doulas and 32 Village based social workers have been trained and are practicing in villages of Aoral and Phnom Sruoch districts of Kampong Speu province.

3. Objectives & Assignment of Services

The specific objective of this assignment is to identify, organize, and train community interest groups in selected communes and villages of the districts of Aoral, Phnom Srouch and Thpong (Choung Rok Samaki commune) into sustainable community based organizations (CBOs) through a participatory approach and ensure these groups have a basic set of skills to function independently.
Training will specifically focus on the development of the ability to do needs assessments, plan, network, and fundraise for community initiatives, implement activities, and monitor/evaluate the impact of the interventions.

The Consultant will work through the project field bases of E&D in collaboration with project staff and relevant stakeholders. The Consultant will provide weekly feedback of progress in summary format to the SRH Project Coordinator and will discuss issues/constraints in delivering these ToRs.

4. Methodology
The Consultant will prepare an activity plan in close consultation with project staff. For Organization of community groups the Consultant will identify existing structures wherever possible. He/she will describe the structures for decision-making and the mechanisms for articulating community and individual needs and preferences. Structures/ mechanisms may be formal or informal but necessarily participatory, with operating guidelines and procedures. Special interest groups (e.g. doulas, social workers, and youth) will be organized to ensure sustainability of interventions of the village volunteers.

Community based organizations will be empowered to represent the community, allowing them to influence key decision-makers on a range of issues over time.  It is envisioned that this process will lead to the identification and/or development of new local leaders, facilitating networks and assisting in the development of action plans.

Key elements for development of the CBOs will be capacity building, community leadership and community organizational development. The consultant will facilitate the process through participatory involvement of interest groups, existing community leaders, and key project staff.

5. Time frame
The duration of the services will be for approximately 4 weeks and the expert will ensure that the objectives are achieved according to a Work Plan designed in collaboration with the project Prevention Coordinator and other staff.

6. Expected Outputs
A completed activity plan and a final comprehensive report demonstrating the setup and development of basic skills of CBOs in selected communes. The skill set will include the capacity to do participatory needs assessment, activity planning, networking, and fundraising for community initiatives. Additionally, the CBOs will have basic skills in implementing activities, and monitoring/evaluating the impact of the interventions.

The report will be submitted to and approved by the SRH Prevention Coordinator with recommendations, within one week of completion of the field assignment.

7. Proposed qualifications of Consultant 
  • Sound knowledge and experience of community organizing, community development and empowerment.  
  • Demonstrated experience in organizational development. 
  • Demonstrated experience in doing training needs assessments, developing and delivering community capacity building programs.
  • Good technical drafting and report writing skills in English and strong presentation and interpersonal communication skills 
  • Working knowledge of Khmer language will be an asset.
